Our Policies

• Cash pay is an option as well. All copays, balances and cash payments are due on the day of service.  

• It is the patient’s responsibility to make sure we are in your insurance network, to be aware if you have a deductible and to make payments in a timely manner. 

• Appointments will not be made if there is a balance of $100 or more.  

• Cancellations within 24 hours of your scheduled appointment will incur a $100 cancellation fee

• All intake paperwork must be completed, signed and returned prior to scheduling the initial appointment. This ensures time to receive, upload and make sure all documents needing signatures are signed.  This also gives the provider enough time to prepare and review your history prior to seeing you.  If it is not received in time, you will risk a cancellation fee and rescheduling.
